Clinical Coordinator - Radius Home Care

This role sits within Radius Care’s Homecare services, supporting clients to live safely and independently in their own homes.
Location: Parnell, Auckland. We are open to candidates that live in Hamilton or Christchurch.
Hours of work: 20 hours per week

About the role:
As our Clinical Coordinator, you’ll oversee the delivery of safe, responsive care for clients in their homes. You’ll coordinate services, support and develop Support Workers, and work closely with clients, whanau, and external health professionals to ensure excellent outcomes.
This role combines clinical oversight, service coordination, leadership, and customer experience, with the opportunity to help grow a greenfield service.
What you’ll be doing:
Assess clients’ needs and develop personalized care plans
Provide clinical oversight and monitor client outcomes
Coordinate care delivered by Support Workers and clinical staff
Act as a key point of contact for clients, whanau, and referrers
Support recruitment, onboarding, induction and development of staff
Work with training and people teams to ensure capability and compliance
Ensure documentation, audits, and incident management are completed to a high standard
Build strong relationships with GPs, community teams, ACC and other providers
Contribute to service growth, quality improvement, and workforce planning
You’ll bring:
A degree in Nursing and valid APC
Experience in home care or community care (leadership experience preferred)
Strong clinical judgement and decision‑making skills
A calm, positive approach when managing competing priorities
Clear communication skills with clients, whanau, and staff
Confidence using systems and managing clinical documentation
