Service Coordinator - Radius Home Care

Location: Radius Support Office, Level 4, 56 Parnell Road, Parnell, Auckland
Hours of work: 30 hours per week

About the role:
This role is the crucial link in our service delivery chain: from receiving referrals, to matching the right Support Worker with the right client and keeping everyone informed along the way. You’ll play a central part in ensuring people receive timely, personalised care that allows them to live safely, independently, and with dignity in their own homes.
You’ll work closely with clients, whānau, Support Workers, and clinical teams to make sure the right care is delivered at the right time.
Key Responsibilities:
- Receive, process, and prioritise referrals
- Match clients with Support Workers based on needs, skills, and availability
- Manage schedules and rostering to ensure continuity of care
- Keep clear communication flowing between clients, families, staff, and clinical teams
- Monitor service delivery and resolve issues as they arise
- Maintain accurate records across digital systems and databases
- Support cultural responsiveness and equity in all aspects of care
About You:
- Organised and detail-focused, with strong time management skills
- A natural communicator, empathetic yet professional
- Tech-savvy and comfortable using Microsoft Office, scheduling systems and AI
- A quick thinker and problem-solver who can stay calm under pressure
- Culturally aware and committed to equity and inclusion
- Experienced in healthcare, administration, or customer service (clinical experience is an advantage but not essential)
