Hours of work: 20 hours per week

About the role:
We are seeking a registered health professional to join our team as a Clinical Coordinator.
This role combines service coordination with clinical oversight. You’ll be responsible for receiving and processing referrals, matching the right Support Worker with the right client, and keeping everyone informed – while providing clinical input, supporting safe practise, and ensuring high quality care delivery.

Key Responsibilities:
- Match Support Workers to clients, ensuring safe skill and competency alignment.
- Provide clinical oversight for care plans and risk management where required.
- Act as a clinical point of contact for Support Workers and clients with complex needs.
- Monitor service delivery, escalate clinical issues, and support continuous quality improvement.
- Maintain accurate documentation in line with professional and contractual standards.
- Foster strong communication between clients, whānau, Support Workers, and clinical teams.
- Clinically review incidents, escalate where necessary, and support learning/improvement processes.
- Contribute to onboarding, mentoring, and clinical competency checks of Support Workers.
- Participate in internal audits, reporting, and quality improvement initiatives with a clinical lens.
About You:
- A Registered Nurse (or other relevant health professional) with a current Annual Practising Certificate
- Experienced in community & homecare settings
- Skilled in service coordination, rostering, or care planning
- Organised, detail-oriented, and able to manage competing priorities
- A natural communicator with excellent relationship building skills
- Empathetic, culturally aware, and committed to equity and inclusion
- Confident using Microsoft Office and digital care/scheduling systems
- Organized and detail-focused, with strong time management skills
- A natural communicator, empathetic yet professional
- Tech-savvy and comfortable using Microsoft Office, scheduling systems and AI
- A quick thinker and problem-solver who can stay calm under pressure
- Culturally aware and committed to equity and inclusion
- Experienced in healthcare, administration, or customer service (clinical experience is an advantage but not essential)
